How to change your name after marriage in Oklahoma
To change your name after marriage in Oklahoma, update Social Security, then notify the Department of Public Safety in writing within 10 days. Request a duplicate license in person at a Licensed Operator or Service Oklahoma office; it can't be done online. The duplicate costs $25. Service Oklahoma asks for originals or certified copies and doesn't accept notarized documents; for a common-law marriage, the Department of Public Safety asks for an affidavit with both spouses' notarized signatures.

Each document must be an original or a certified copy. Service Oklahoma says notarized documents are not accepted.
For a common-law marriage, the Department of Public Safety asks for an Affidavit of Common Law Marriage with the notarized signatures of both spouses (see What to bring).
Do I need to update Social Security before going to Service Oklahoma?
Not confirmed yet. Service Oklahoma's page we checked doesn't say. Several other states do ask for Social Security first, so it's a safe order to follow. Check Service Oklahoma's link in step 3.
What documents does Service Oklahoma need to change my name after marriage?
The Oklahoma Department of Public Safety's name-change sheet lists: primary ID (usually a state-certified birth certificate), secondary ID (usually a Social Security card), marriage license, divorce decree, adoption decree or name change decree and for a common-law marriage: an Affidavit of Common Law Marriage. Each document must be an original or a certified copy. Service Oklahoma says notarized documents are not accepted.
